The UBR-Fxx.TXT file contains five fields separated by a tab.

Field 1 Revenue Code: standardized four digit revenue code. The X in the fourth 
position denotes the subcategory and must be replaced by the appropriate 0-9
code on a claim. 0 denotes a "general" category in most cases and can be used 
instead of more specific subcategories. Providers are recommended to be as
specific as possible however since payers may delay claim processing with out 
fully specified subcategories.

Each service should be assigned a revenue code:
1) For inpatient services that have multiples of the same item, providers 
   should aggregate the services under one revenue code and report the total
   units.
2) For outpatient services, providers should report corresponding HCPCS codes.
3) For multiple HCPCS services on the same day, providers should aggregate the
   services unless they are evaluation and managment codes. In this case, each
   should be reported separately with Condition Code G0 to indicate a distinct
   medical visit.
4) Services provided on different days should be listed separately with date of
   service, units, and revenue code.
5) Revenue codes should be listed in ascending numeric order by date of service
   for outpatient claims although rev code 0001 is reported on Line 23 of the 
   last page of the claim.

Field 2 Name: Full length name

Field 3 Abbreviation: intended for Charge Description Master and should not be
        reported on electronic claims

Field 4 Units: what the revenue code services are measured in

Field 5 HCPCS: General guidance on whether HCPCS are applicable to a particular
               revenue code. These are not absolute rules.

Due to HIPAA standardization of medical billing there is now consistency among 
CHAMPUS, Commercial insurance, Blue Cross, Medicare, and Medicaid.
