Field 3 CATEGORY - Item category code
Field 4 REGION - Full name of region to which the DME fee schedule entry applies
Field 5 MODIFIER_ONE - First modifier specifying the nature of the HCPCS coded item 
Field 6 MODIFIER_TWO - Second modifier specifying the nature of the HCPCS coded item 
Field 7 CEILING - Maximum fee schedule amount for the item.
Field 8 FLOOR - Minimum fee schedule amount for the item.
Field 9 FEE - Fee amount for the item for the specified region
